About The Position

Terros Health is seeking a Crisis Case Manager - Peer Aftercare Specialist for its Central Road location. This role involves providing behavioral health crisis services to children and adults, helping them understand their crisis within their recovery goals and develop coping skills. The Peer Aftercare Specialist will assist patients and their families in navigating illness and injury, from hospitalization to recovery, similar to a hospital social worker.

Requirements

  • GED/high school diploma with a minimum of 1.5 years of behavioral health experience, OR an associate’s degree in the behavioral health field, OR a bachelor's degree (not in the field of behavioral health) and 1 year of behavioral health experience, OR a bachelor's degree in the behavioral health field (e.g. Psychology, Social Work, Counseling) or Masters degree, i.e., MA, MC, MSW, in a behavioral health field
  • Arizona license or eligible for Arizona license
  • Peer Certification within 6 months of hire date
  • Self-identified, first-hand experience with self or family members with serious mental illness and/or a chronic disease
  • Utilize a recovery approach and philosophy
  • Able to liaison between multiple parties
  • Exemplify great customer service and motivational skills
  • Must have or be able to attain CPR/First Aid certification within 60 days of assuming role
  • Excellent written and verbal communication skills
  • Strong interpersonal skills
  • Ability to work on a team
  • Proficient in computer use and applications such as Microsoft Office Suite
  • Must have or obtain a valid Arizona Driver License
  • Be 21 years of age with a minimum 4 years driving experience
  • Must meet requirements of Terros driving policy
  • Must have a valid Arizona Fingerprint Clearance card (Level 1) or apply for an Arizona Fingerprint Clearance card within 7 working days of employment
  • Must pass a TB test

Nice To Haves

  • Certified Case Manager (CCM)
  • Familiar with filing mental health petitions (or willing to learn)
  • Clinical documentation experience preferred
  • Working with Electronic Health Records (EHR) systems highly valued

Responsibilities

  • Direct interactions and service delivery for patients
  • Accessing community-based services to assist patient’s transition
  • Ensure linkage to community services and resources
  • Float between crisis department aftercare programs: Bridging the Gap Crisis, Crisis Transition Navigator, and Community Stabilization Team, based on departmental need.
