About The Position

The Care Manager – Crisis Services provides person-centered care coordination and support to individuals experiencing behavioral health crises. Working within a fast-paced crisis services environment, this position engages clients at critical points of need to assess risks, identify strengths, coordinate services, and facilitate timely access to appropriate levels of care. The Care Manager collaborates closely with clinical, medical, and community partners to support stabilization, continuity of care, and recovery-oriented outcomes.

Requirements

  • Master’s Degree in a human services related field OR Bachelor’s degree in a human services related field and 1 year relevant experience working with the target population preferred OR High School Diploma or GED with a minimum of 3 years of experience working with the target population
  • Strong understanding of mental health and substance use disorders, harm reduction, and recovery models.
  • Knowledge of community resources, social service systems, and healthcare navigation.
  • Ability to build rapport, collaborate effectively, and maintain professional boundaries.
  • Strong organizational skills, follow-through, and ability to manage multiple priorities.
  • Cultural humility and ability to deliver services with respect to diverse identities and backgrounds.
  • Proficient in documentation, electronic health records, and coordinated communication across providers.
  • Valid driver’s license

Responsibilities

  • Engage individuals experiencing acute behavioral health or substance-related crises using trauma-informed, recovery-oriented, and culturally responsive approaches.
  • Conduct care management activities including screening, needs identification, service planning, and coordination across systems of care.
  • Support crisis stabilization by facilitating referrals to appropriate services such as outpatient care, inpatient hospitalization, residential treatment, detoxification, or community-based supports.
  • Coordinate transitions of care to reduce gaps in services and prevent unnecessary hospitalizations or repeated crisis episodes.
  • Collaborate with clinical staff in gathering information related to risk, safety, social determinants of health, and service needs.
  • Assist with the development and implementation of individualized service or care plans in coordination with the interdisciplinary team.
  • Maintain timely, accurate, and compliant documentation in electronic health records and other required systems.
  • Work closely with crisis clinicians, peer support specialists, medical providers, law enforcement, emergency departments, and community partners to coordinate care.
  • Serve as a liaison between clients, families (as appropriate), providers, and funding sources.
  • Advocate for client needs while maintaining professional boundaries and ethical standards.
  • Support clients in understanding available resources, treatment options, and next steps following a crisis episode.
  • Promote client self-determination, dignity, and empowerment during and after crisis interventions.
  • Provide follow-up outreach as assigned to support engagement in ongoing services.
  • Adhere to all agency policies, licensing standards, and applicable state and federal regulations.
  • Participate in team meetings, trainings, and quality improvement initiatives.
  • Maintain confidentiality and ethical standards in accordance with professional guidelines and HIPAA.
  • Additional responsibilities as needed to support program operations.
