About The Position

Imagine Pediatrics is a tech-enabled, pediatrician-led medical group dedicated to improving care for children with special healthcare needs. They offer 24/7 virtual-first and in-home medical, behavioral, and social care, collaborating with families, providers, and health plans to overcome access barriers. The role enhances existing care teams by providing compassionate, creative support for children with medical complexity. This position is primarily remote with a shift work schedule (Mon-Fri 10:30 am - 07:00 pm CST). As a Crisis Team Case Manager, you will support families of behaviorally and medically complex children by offering emotional support, care coordination, resource connection, and assistance navigating the healthcare system after hospital discharge. The centralized crisis team includes therapists, case managers, and a transition of care coordinator.

Requirements

  • Master’s degree in Social Work (MSW) Required
  • 2+ years of case management or relevant experience
  • Experience with team-based care, excellent collaboration skills.
  • Experience in child & adolescent case management, school-based care, inpatient setting, and/or community mental health.
  • Experience with acute behavioral health needs (suicidality, homicidality, severe persistent mental illness).
  • Excellent working knowledge of community resources

  • Bilingual in English/Spanish preferred

Responsibilities

  • Providing support and encouragement to patients and families who are experiencing / recently experienced a behavioral health crisis.
  • Handle case referrals, assess for resourcing needs and care gaps, review case progress, and determine case closure.
  • Assist patients and families in their individual recovery goals through psychoeducation and linkage to community resources.
  • Assess social determinants of health and barriers to care including but not limited to housing, income, food insecurity, and transportation needs.
  • Connect patients and families with support groups, community networks, and aids toward mental wellness.
  • Collaborate with interdisciplinary team members to facilitate the patient and family’s treatment plan goals.
  • Coordinate care management with agencies surrounding the child such as DCFS, school systems, outside care team members, and more.
  • Participate in ongoing scheduled consultations with an interdisciplinary team to monitor patient progress.
  • Perform other duties as assigned.
