About The Position

This role will provide crisis care coordination to individuals that are identified as requiring a period of psychiatric stabilization (one to six months). Individuals are identified by Crisis Services to be in need of further stabilization, and/or awaiting placement into a long term level of care. Services are provided in the community, i.e. at the person's residence, doctor's clinic, etc.

Requirements

  • Current Resident of Wisconsin.
  • Possession of a valid Wisconsin Driver’s License, able to be maintained throughout employment.
  • Possession of a Bachelor’s Degree or higher in Human Services, Psychology, Social Work, or Occupational Therapy.
  • At least three (3) years of Human Services experience.
  • Intermediate knowledge of all related computer and software applications, such as word processing and spreadsheets.
  • Willing and able to travel up to 75% of the time in this role.

Nice To Haves

  • Possession of Licensed Professional Counselor (LPC) or Licensed Clinical Social Worker (LCSW) licensure.

Responsibilities

  • Conduct an individual's assessment and crisis plan and collaborate with referral sources.
  • Develop individualized client care plans that are measurable under DHHS 34 state guidelines.
  • Document progress notes that promote a person's recovery goals.
  • Deliver Crisis Care Coordination to individuals until formal services can be obtained or the consumer has stabilized. This may include the following: assistance with benefits, linkage to other services, discussion of stressors, including documenting as outlined in state guidelines.
  • Attend any necessary staffing, community, or advocacy meetings.
  • Complete warm handoff to a long-term agency that will be serving the individual when necessary.
  • Perform all other related duties as assigned.
