About The Position

Lighthouse Behavioral Wellness Centers is seeking a Coverage Care Coordinator to help guide individuals through mental illness, trauma, or addiction. This role focuses on empowering, uplifting, and healing clients, particularly those who have recently experienced crisis or hospitalization. The Coverage Care Coordinator will assist clients in navigating multiple systems, engaging with new treatment teams, and adjusting to changes outside the clinical setting. The position requires team members to understand and meet expected performance standards, with Lighthouse providing necessary tools and training for success.

Requirements

  • Bachelor or master’s degree in education, Psychology, Social Work, Sociology, Criminal Justice or similar field approved by ODMHSAS; OR
  • A current license as a registered nurse in Oklahoma with behavioral health experience; OR
  • A bachelor’s or master’s degree in any field with 6 months of behavioral health experience; OR
  • 60 college credit hours in any field and 12 months of behavioral health experience; OR
  • A high school diploma or equivalent and a total of 36 months of direct, documented experience working with persons who live with mental illness and/or substance use issues.
  • Applicants must be able to provide documented proof from previous employers to justify behavioral health experience.
  • Current certification as a Behavioral Health Case Manager II as described in ODMHSAS Standards ad Criteria for Certified Behavioral Health Case Managers 450:50-3-1
  • An expectation of core competencies in relation to individuals and families, as well as individuals with co-occurring disorders, trauma-informed care and collaborative care.
  • Working knowledge of healthcare eligibility, application processes, renewals, and compliance requirements, as well as familiarity with other public assistance programs (e.g., SNAP, SSI/SSDI, housing supports).
  • High level of accuracy in completing applications, tracking documentation, and ensuring compliance with regulatory requirements.
  • Strong ability to manage multiple cases, track deadlines (e.g., Healthcare renewals), maintain documentation, and prioritize tasks effectively.
  • Understanding of integrated care models, CCBHC framework, care coordination practices, and service delivery across physical, behavioral, and community health systems.
  • Knowledge of person-centered planning, strengths-based approaches, trauma-informed care, and systems of care for children and families.
  • Ability to work effectively with diverse populations, respecting cultural values, preferences, and individual circumstances.
  • Skilled in working collaboratively with internal teams, external providers, and legal partners to ensure cohesive, integrated service delivery.
  • Strong verbal and written communication skills, with the ability to explain complex processes in a clear, supportive, and culturally responsive manner.
  • Understanding of ODMHSAS standards and confidentiality laws (HIPAA), and accurate recordkeeping practices.
  • Ability to guide clients through complex systems, troubleshoot application issues, prevent coverage gaps, and resolve barriers to accessing services.
  • Must meet “satisfactory” expectations at any scheduled job performance evaluation, coaching and/or Performance Improvement Plans.
  • Assessment and appraisal of quality of work will be evaluated through training, skill, chart review or special purpose.
  • Must possess skill in working cooperatively and effectively with clients, staff, management and other professional and community groups; in exercising mature judgment in dealing with people; in presenting ideas clearly and accurately; in reading and comprehending the English language; and in communicating effectively, both orally and in writing.

Responsibilities

  • Provide comprehensive, client-centered care coordination and targeted support services.
  • Assist with Healthcare enrollment, application follow-up, eligibility tracking, and navigation of additional healthcare and social service benefits.
  • Deliver case management, care coordination, and coverage navigation based on client needs and goals.
  • Assist individuals in accessing, understanding, and maintaining Healthcare coverage and other available benefits.
  • Ensure continuity of care and reduce barriers to services.
  • Meet with new clients to complete Healthcare applications.
  • Monitor application status.
  • Manage renewal and expiration timelines.
  • Support re-enrollment efforts to prevent gaps in coverage or support.
  • Link and advocate for legal assistance as needed by working collaboratively with individuals, care team, and attorneys.
  • Track and update legal assistance referrals, including final findings and actions.
  • Complete Lighthouse required training and resulting follow up and consultation as required at hire, annually and as directed by Supervisor.
  • Perform other reasonably related duties as assigned by the immediate supervisor or other management as requested.
