About The Position

Transitional Services for New York, Inc. Responsibilities

  • Provide direct care services to an assigned caseload of residents.
  • Assist residents in learning and practicing daily living skills, including personal hygiene, laundry, budgeting, and room maintenance.
  • Accompany residents to medical appointments, benefits appointments, and off-site recreational activities.
  • Conduct individual weekly sessions with assigned caseload.
  • Manage clothing inventory and assist with purchasing seasonal clothing.
  • Converse with the outgoing shift for pertinent information regarding resident appointments, behaviors, and medication changes.
  • Supervise morning, afternoon, and evening medication administration and immediately initial the medication book after each dose.
  • Alert day programs of resident absences or lateness due to appointments.
  • Complete a change of shift checklist, including medication counts, resident counts, chart counts, agency vehicle location, house keys count, and inventory of valuables.
  • Check staff revised schedules and personal mailboxes.
  • Assist residents with meal preparation as needed.
  • Report pertinent information regarding residents and the facility to the shift supervisor.
  • Document pertinent resident information in the Communication Log.
  • Meet with assigned caseload as scheduled for discussions on outstanding and ongoing issues.
  • Document all sessions on progress notes.
  • Maintain resident charts in accordance with Agency and OMH requirements.
  • Meet with supervisor as scheduled.
  • Ensure all caseload residents have sufficient medication.
  • Escort residents to appointments and for personal shopping needs.
  • Write Medicaid Progress Notes reflecting restorative services.
  • Complete and forward status change forms immediately upon hospitalization or other status changes to the Program Director.
  • Visit hospitalized residents weekly.
  • Participate in and supervise residents during fire drills and Community Meetings.
  • Participate in staff meetings.
  • Complete quarterly ADL assessments for all caseload residents.
  • Participate in quarterly service plan review assessment and development.
  • Inspect residents' rooms for safety measures and report any damages or problems.
  • Escort residents to the Social Security office and for clothing shopping.
  • Complete functional assessments.
  • Ensure residents have had their annual physical and dental check-ups.
  • Submit receipts for petty cash spent.
  • Inform residents of appointments.
  • Conduct and document room checks as needed.
  • Attend special on-site and off-site training, seminars, and conferences.
  • Check revised schedules for changes in hours/shifts and holiday coverage.
  • Assist residents in completing Home Leave forms.
  • Ensure residents are dressed appropriately before leaving the facility.
  • Supervise kitchen activities to ensure safety, health, and cleanliness.
  • Assume Med/Shift Supervisor responsibilities as scheduled/needed.
  • Perform other related duties as required.
