About The Position

Brentwood Hospital is seeking a dynamic Counselor / Social Worker to join its team of professionals dedicated to improving mental health for individuals and families. The role involves providing a safe, nurturing environment and offering options for continued improvement. The LPC/LCSW or LMSW/PLPC will deliver quality case management and psychotherapeutic services to adult psychiatric patients (18+), adult chemical dependency patients (18+), adolescent patients (13-17), and child patients (4-11), as well as their families and significant others. This position serves as a member of an interdisciplinary team, supporting the organization’s treatment program and philosophy to ensure quality treatment. Brentwood Hospital is a 260-bed psychiatric and chemical dependency treatment facility in Shreveport, Louisiana, with over 45 years of service to Louisiana, East Texas, and South Arkansas, providing individualized treatment to children, adolescents, adults, and seniors. Requirements

  • Master's Degree in social work, counseling or related field
  • LPC/ PLPC or LCSW/LMSW license required

Nice To Haves

  • Experience in inpatient or outpatient mental health setting preferred

Responsibilities

  • Conducts psychosocial assessments to document history and identify preliminary issues for treatment focus by interviewing the patient and their family members.
  • Provides group and family process-oriented therapies as well as conducts educational and other didactic groups for patients and family members using various professional treatment modalities.
  • Daily progress notes summarized patient’s progress toward treatment goals; Contact with family, significant others, community agencies recorded in progress notes.
  • Develops and coordinates an individualized discharge plan for the patient by utilizing treatment team and chart information to determine the patient’s aftercare needs. Identifies and assesses family or community resources such as, group homes, or mental health practitioners to meet patients’ aftercare needs.
