About The Position

This role involves providing comprehensive counseling and support services to consumers, focusing on substance abuse and mental health. The position requires conducting screenings, assessments, developing service plans, providing individual and group therapy, and coordinating care with other providers. The incumbent will also be responsible for case closing, aftercare planning, and ensuring adherence to all relevant policies and safety protocols.

Requirements

  • Master’s degree in a related field with clinical experience in Substance Abuse or Mental Health.
  • Certified Addictions Professional (CAP) credential in Florida and/or FL License in Social Work, Marriage and Family Therapy or Mental Health Counseling.
  • Valid Florida Driver License.

Responsibilities

  • Screen potential consumers within 24 hours of referral, prioritizing urgent needs.
  • Complete clinical intake documentation for admissions.
  • Use standardized criteria and assessment instruments to match consumer needs with available services.
  • Identify and address unmet medical needs, including detoxification, medication monitoring, and laboratory testing.
  • Complete clinical assessments within 10 days or sooner as required.
  • Develop service plans within 15 days for residential and 30 days for outpatient consumers, or within 24 hours for those in crisis.
  • Develop service plans with full consumer participation, including agreed-upon goals, desired outcomes, timeframes, services, and signatures.
  • Review and update service plans every 30 days and discuss in clinical staffing.
  • Provide weekly individual therapy and assigned group therapy sessions using evidence-based curriculum.
  • Explore reasons for treatment and sober living in individual and group sessions.
  • Identify successful coping strategies for staying sober.
  • Provide trauma-based care techniques for victims of abuse, neglect, assault, or other violence.
  • Evaluate progress towards goals and modify service plans as appropriate.
  • Provide educational sessions based on Hazelden curriculum.
  • Coordinate care with other service providers with consumer consent.
  • Participate with the clinical team in decisions regarding level of care, treatment, discharge planning, and follow-up.
  • Enter progress notes into the client record at least weekly for residential consumers or according to session frequency for outpatient.
  • Provide SOAR services in coordination with the interdisciplinary team.
  • Adhere to No Wrong Door Policy and provide or refer for services such as work-related services, job placement, supported housing, transportation, social skills training, budgeting, credit counseling, public assistance, interpersonal communications, activities of daily living, medical/dental care, legal assistance, childcare, and social/cultural/recreational/religious activities.
  • Collaborate with the case management team.
  • Begin case closing at intake and ensure completion of agreed-upon service goals.
  • Notify collaborating service providers upon closing a case.
  • Link consumers discharged prior to program completion to appropriate services.
  • Develop aftercare plans during the 30-day Treatment Plan review, detailing needed services and relapse prevention steps.
  • Follow up by phone 30 and 120 days post-discharge with consumer permission.
  • Adhere to dress code, wear appropriate clothing, and follow safety protocols per OSHA and Catholic Charities.
  • Participate in monthly Fire Drills.
  • Ensure appropriate reporting of incident reports in adherence to DCF and Catholic Charities Policies and Procedures.
  • Comply with all policies, procedures, and requirements necessary to perform the functions of this position.
  • Perform additional responsibilities as required by supervisor.
