About The Position

Western State Hospital, part of the Virginia Department of Behavioral Health and Developmental Services (DBHDS), has been a leader in psychiatric treatment since 1828. WSH is a 302-bed Joint Commission/CMS-accredited psychiatric facility located in the scenic Shenandoah Valley. Just 30 minutes from vibrant university towns, our hospital serves adults with serious mental health needs and is nationally recognized for our Psychosocial Rehabilitation “Learning” programs. Western State Hospital is seeking a full time Counselor II to develop and implement person-centered treatment and discharge plans in collaboration with consumers and other members of the interdisciplinary treatment team and serve as link between community agencies and others that support consumer’s recovery.

Requirements

  • MSW, or MSSW required (graduation from a CSWE accredited undergraduate or graduate social work program)
  • Professional work experience with adults with serious mental illness

Responsibilities

  • Provides SW assessments and identifies discharge needs.
  • Provides PSR interventions and participates in developing and implementing individualized treatment plans.
  • Completes SW assessment within 7 days of admission
  • Develops and maintains partnerships with Community Service Providers
  • Meets with CSB liaisons to discuss discharge and recovery needs
  • Provides link between the WSH treatment team and consumers’ families and other persons that support recovery
  • Alerts families to critical events in collaboration with other team members
  • Provides clinical documentation per requirements and established time frames outlined by Joint Commission, CMS, and SW Department standards
  • Continuous Learning/Development and Participation in Social Work and hospital-wide service delivery system
  • Completes 10 hours of continuing education yearly (CME’s or other formal training, workshops).
  • Completes annual mandatory training within required timeframes
