About The Position
Provides assessment, diagnostic, individual, group and family therapy services to patients treated on an outpatient basis. Formulates individual patient treatment plans and completes all required documentation. Systematically and continually assesses the patient's mental status in collaboration with the patient/family/providers. Provides outpatient individual, group, and family counseling. Identifies and refers patients to appropriate community resources for follow-up care.
