Columbia Community Mental Health is a nonprofit agency and Certified Community Behavioral Health Clinic that provides services to individuals with mental, addiction and developmental disabilities. We are looking for qualified individuals to join our new program and team! We have an open Counselor I position on our ACT Team. The ACT team is a part of our Intensive Behavioral Health Services (IBHS) Department. Summary: The Counselor I is a member of the multidisciplinary Assertive Community Treatment (ACT) team and provides comprehensive, community-based behavioral health services to individuals living with serious mental illness (SMI). Working collaboratively with Case Management, Medical Services, Peer Support, family members, and community partners, the Counselor I assists participants in achieving recovery goals, increasing independence, and reducing the need for hospitalization and institutional care. This Counselor I position also provides administrative support to the ACT team. Clinical responsibilities include providing skills training, crisis intervention, assessment, care coordination, and ongoing support in community settings where participants live, work, and socialize. The Counselor I actively engages participants in treatment, supports medication adherence as appropriate, assists with resource linkage, and helps individuals develop coping, daily living, and community integration skills. The position participates in treatment planning, multidisciplinary team meetings, and outreach efforts to ensure services remain person-centered and recovery focused. Administrative duties include: Facilitation of the day's operations in a supportive manner for the team, clients, natural supports and other ancillary service providers (e.g., landlords, social security). Provide direct support to staff, including monitoring and coordinating daily team schedules and supporting staff both in the office and field. Serve as a liaison between clients and staff such as attending to the needs of office walk-ins and calls from clients/natural supports. Active participation in daily team meetings.
