About The Position

Acenda Integrated Health is seeking a Counselor/Family Intervention Specialist to provide intensive home-based services to families. This role involves completing documentation such as bi-weekly treatment plans, weekly session notes, and ongoing clinical assessments. The position also requires participation in weekly supervision, professional development, group supervision, and clinical case consultation calls. Acenda values the use of evidence-based practices (EBPs) and serves a broad population of youth, including those involved with child-serving systems and at high risk of removal from their families. The program specializes in diverting youth from out-of-home placements by helping families safely maintain youth in their homes. This position requires a flexible and non-traditional schedule, designed around the availability of the youth and families served.

Requirements

  • Bachelor's degree in social services with at least one year of experience in counseling or case management required
  • Ability to facilitate positive interactions and work collaboratively with families
  • Excellent communication, organization, and documentation skills
  • Must have and maintain a valid driver's license, use of an insured vehicle and an acceptable driving record

Nice To Haves

  • Experience working with children and families, particularly families involved with the child welfare system and/or affected by trauma preferred
  • Bilingual preferred
  • Master's degree in field of counseling, psychology or social work

Responsibilities

  • Carry small caseload of 4-6 families
  • Hold family sessions with each family 3 times a week scheduled at the convenience of the families
  • Provide treatment in families' homes, schools and other community settings as needed
  • Participate in weekly supervisions and clinical consultation meetings
  • Provide on-call availability to support families
  • Address behavioral health needs to support family functioning and stability
  • Implement and adjust evidence-based intervention strategies to align with family goals
  • Collaborate with peers, DCP&P, and other community providers to ensure continuity of care and support for the family
  • Complete necessary documentation required for Youth Villages Intercept program and Acenda
