About The Position

Lutheran Social Services of Illinois (LSSI), a leading not-for-profit organization, is seeking a full-time Counselor Case Manager for its Belmont Outpatient SURS program. This role is responsible for providing comprehensive, whole-person services to clients engaged in substance use disorder (SUD) services. Key duties include conducting 1:1 sessions, offering case management assistance, and facilitating evidence-based group programming, which may also encompass DUI services. The Counselor Case Manager will collaborate with clients to complete assessments and treatment plans, and support them in achieving their personal goals and objectives. Requirements

  • High school diploma or equivalent required.
  • CADC or the ability to obtain a CADC within 18 months of hire date required.
  • Demonstrated proficiency in Electronic Health Record documentation and review required.
  • Familiarity with accreditation and licensure standards required.
  • Demonstrated ability to work successfully independently as well as in a team environment.
  • Demonstrated ability to communicate in a clear, comprehensible manner, both verbally and in writing.
  • Excellent organization, presentation, and pc/computer skills, including experience using Microsoft Office (Outlook, Teams, Word, Excel, PowerPoint) along with other related software.
  • Valid driver’s license, in good standing for the state of residency required.
  • Access to reliable transportation required.
  • Valid IL statutory minimum liability insurance coverage, bodily injury, and property damage required.
  • Background check clearance required.
  • Annual TB test required.

Nice To Haves

  • Associates, bachelors, or master’s degree in a social service field is preferred.
  • LPC/LCPC or LSW/LCSW preferred.
  • If masters-level, work toward clinical licensure.
  • Experience establishing and maintaining collaborative community partnerships preferred.
  • Experience working with clients of all ages, with varying issues and symptoms preferred.

Responsibilities

  • Provide whole-person services to clients participating in substance use disorder (SUD) services.
  • Conduct 1:1 sessions with clients.
  • Provide case management assistance.
  • Present evidence-based group programming.
  • Provide DUI services as needed.
  • Collaborate with clients to complete assessments and treatment plans.
  • Assist clients in achieving their stated goals and objectives.
  • Meet and exceed revenue productivity targets.
  • Carry client caseload to meet 60% productivity (25 hours/week).
  • Understand and follow all compliance rules regarding treatment services and contracts.
  • Understand and reflect the clinical model of the program.
  • Adhere to all timelines regarding documentation completion.
  • Work with Supervisor on building clinical skillset.
  • Ensure adherence to ethical standards of treatment, including NASW Code of Ethics.
  • Demonstrate professional, positive behavior and carry out responsibilities with integrity, treating clients, families, other LSSI workforce members, and collaborative organizations and/or individuals in a dignified, respectful, honest, and fair manner.
