About The Position

The Geriatric Mental Health Outpatient Service (GMHOS) is seeking a full-time, permanent, Counselor Therapist 2 to work on the Geriatric Mental Health Outreach team (GMHOT). The incumbent will provide direct clinical care for adults who are experiencing addiction or concurrent disorder who are living in long term care. Accountabilities include addictions support and education for both clients and long term care home staff members. The role involves conducting assessments from received referrals for new clients, treatment and care planning with the physician, and engagement with family members/community supports and long term care. They will also be a resource to the GMHOT team with guidance and support around addictions in geriatric populations. The incumbent will be able to work with clients from diverse populations, and demonstrate a harm reduction, recovery-oriented and trauma-informed approach to care. The incumbent will also participate in: continuous quality improvement initiatives; the research and educational activities of the program; program development; service provider relationship-building; and capacity-building activities. Other related duties may be assigned as required.
