The Activities Coordinator is responsible for fostering a vibrant, engaging, and resident-centered environment by facilitating meaningful activities and social opportunities within each household. This role empowers residents to pursue personal interests, build relationships, and participate in purposeful daily life experiences that enhance overall well-being. Requirements

  • Is licensed or registered, if applicable, by the State of Minnesota
  • Eligible for certification as a Therapeutic Recreation Specialist or Activities Professional by a recognized accrediting body
  • Has two (2) years of experience in a social or recreational program within the last five (5) years, with at least one (1) year full-time in a therapeutic activities program
  • Is a qualified Occupational Therapist (OT) or Occupational Therapy Assistant (OTA)
  • Has completed a State-approved training course.
  • Successful completion of nurse aide training within 6 months of hire
  • Completion of activities professional certification within 1 year of hire

Nice To Haves

  • Previous experience in geriatric or long-term care settings preferred
  • Supervisory or leadership experience preferred
  • Strong planning, documentation, and organizational skills
  • Ability to work collaboratively in an interdisciplinary, team-based environment

Responsibilities

  • Works cooperatively and respectfully with residents, coworkers, families, visitors and volunteers to create a positive and pleasant environment in the home. This may involve direct problem solving, conflict resolution and ensuring that activities are meeting the needs and desires of residents.
  • Planning and coordinating activities.
  • Completing RAI documentation and gathering information regarding activity needs to create resident plan of care.
  • Attend care conferences and interdisciplinary meetings per DON direction.
  • Assist resident with activities that are meeting the needs and desire of the residents.
  • Promoting the residents’ well-being and satisfaction by supporting resident-centered care, participating in the interdisciplinary team as it establishes and implements the residents’ plan of care with the resident/resident representative.
  • Participates in a quality work team by promoting teamwork to provide resident-centered care, participating in team problem solving methods and team/individual successes. Including participating in supportive orientation and training of new staff.
  • Maintaining a safe, comfortable and functional environment by assessing the household for potential hazards, utilizes appropriate techniques regarding sanitation, infection control, hazardous waste management, fire safety, disaster preparedness and emergency care.
  • Maintaining a “home” in the physical environment, maintaining a sense of welcoming through enhancing relationships in the household through shared activities of living, promoting compassionate services among the team by offering emotional, psychological and spiritual support to all, addressing the successes and needs of the households.
  • Coordinating/recruiting volunteers
  • Dementia Sensitivity: able to demonstrate and nurture care practices for persons with dementia that meet needs while promoting individualized person-center care and sense of well-being.
