About The Position

The Corrections Linkage and Education Coordinator (CLEC) works with individuals living with HIV and/or Hepatitis C who are incarcerated and/or preparing to return to the community. The CLEC works to motivate and ensure engagement and adherence in treatment, education, prevention, and support services. The CLEC also facilitates Peer Education training and support, targeted recruitment, condom promotion and education, and linkage to testing, treatment, and prevention with incarcerated individuals in designated state correctional facilities. Work with individuals who are living with HIV and/or Hepatitis C who are incarcerated and/or returning to the community in order to motivate and ensure engagement and adherence in HIV and/or Hepatitis C-related treatment, prevention and support services. Service targets will vary by correctional facility; assigned interventions will be outlined in annual workplans.

Requirements

  • Experience working with individuals living with HIV and/or Hepatitis C.
  • Experience working with incarcerated individuals or those returning to the community.
  • Ability to motivate and ensure engagement and adherence in treatment, education, prevention, and support services.
  • Skills in facilitating training and support.
  • Experience in targeted recruitment.
  • Experience in promotion and education.
  • Ability to link individuals to testing, treatment, and prevention services.

Responsibilities

  • Motivate and ensure engagement and adherence in treatment, education, prevention, and support services for individuals living with HIV and/or Hepatitis C who are incarcerated and/or preparing to return to the community.
  • Facilitate Peer Education training and support.
  • Conduct targeted recruitment.
  • Promote and educate on condom use.
  • Link incarcerated individuals to testing, treatment, and prevention services.
  • Work with individuals living with HIV and/or Hepatitis C who are incarcerated and/or returning to the community to motivate and ensure engagement and adherence in HIV and/or Hepatitis C-related treatment, prevention and support services.
  • Meet service targets which will vary by correctional facility and assigned interventions outlined in annual workplans.
