About The Position

This position will serve as a Maternal Health Home Visitor for the Mobile Rural Health Transformation Program. This position provides community-based home visiting services to pregnant and postpartum individuals, with a focus on improving maternal and infant health outcomes. Responsibilities include conducting home visits; providing education on pregnancy, childbirth, postpartum recovery, infant care, breastfeeding, and safe sleep; screening for social and health-related needs; connecting families with community resources and healthcare services; promoting timely prenatal and postpartum care; and supporting participants in navigating the healthcare system. This position will work collaboratively with healthcare providers, community partners, and the program team to address barriers to care, improve health equity, and strengthen maternal and child health outcomes throughout Harford and Cecil Counties.

Requirements

  • Graduation from an accredited high school or possession of a high school equivalency certificate.
  • One year of professional experience in assisting in the coordination of a health-related program(s) or referring people to governmental and private resources.
  • Candidates may substitute 30 college credit hours from an accredited college or university for the required experience.
  • Candidates may substitute U.S. Armed Forces military service experience as a commissioned officer in Health Care Administration classifications or Civil Affairs Specialist specialty codes in the health-related field of work on a year-for-year basis for the required experience and education.
