About The Position

The Coordinator Quality Improvement, Registered Nurse will provide leadership and vision for sepsis and other patient populations in terms of operation, services, and patient care. This role involves concurrent review of medical records against pre-established criteria/quality initiatives, developing and maintaining collaborative partnerships with physicians, staff, and peers to support implementation and ongoing evaluation of care delivery, and overseeing and facilitating care and follow-up of patients. The coordinator will communicate findings of case reviews and abstractions to the team, analyze data, and suggest opportunities to improve patient care delivery. They will develop and sustain plans for monitoring Sepsis and other identified populations, lead and direct achievement in the provision of quality of care for identified patient populations, and analyze data to coordinate the implementation of practice changes. This role also involves maintaining related databases, entering raw data into electronic databases, retrieving data for analysis, and researching and producing internal and external organizational benchmarking reports and studies to identify and measure continuous improvement priorities and targets. The coordinator will review medical records against pre-established criteria for Sepsis and other identified populations, participate in and provide education and clinical care delivery to meet goals, and participate in the development of best practices per the standard of care. Requirements

  • Associates Degree graduate from an accredited school of nursing required
  • 3 years of clinical nursing experience in an acute care setting required
  • RN – Registered Nurse upon hire required

Nice To Haves

  • Bachelors of Science in Nursing preferred
  • Master’s degree in a health related field preferred
  • 1 year of experience in healthcare quality/performance improvement preferred
  • CPHQ – Certified Professional in Healthcare Quality upon hire preferred

Responsibilities
