About The Position

The Post Hospital Discharge Scheduler is the contact person for the inpatient case managers/coordinators to assist in scheduling all PCP and specialist appointments for qualified patients. In addition, the Scheduler will be outreaching to patients to schedule appointments and to remind patients of their upcoming appointments. The Scheduler must be very organized, be efficient, be professional and prompt when interacting with other departments and patients. The Scheduler is also detailed in obtaining medical records and tracking hospital discharges and appointments.

Requirements

  • High School diploma.
  • Computer literate with knowledge and experience with specific software such as Word, Excel, Access.

Nice To Haves

  • Familiarity with managed care, commercial and point of service medical care preferred.

Responsibilities

  • Thoroughly, accurately and timely track hospital discharges to determine which patients are required to have appointments at the Post Hospital Discharge Clinics.
  • Interact with inpatient case managers and coordinators to assist in making appointments for qualified patients with all PCP & specialists post discharge.
  • Work with inpatient coordinator to help make reminder phone calls the day prior to appointments.
  • Review internal systems for medical records and upload them to the Electronic Medical Record to be access by the clinician during the appointment.
  • Assist in rescheduling appointments for patients who cancel or do not show to their scheduled appointments.
  • Maintain excellent tracking of project, process and scheduling.
  • Be proficient at Excel as all of the tracking is done in Excel.
  • Filing, faxing, copying and scanning of files.
  • Strive to meet personal, departmental and organizational goals and targets.
  • Ability to manage a heavy workload with frequent interruptions and changing priorities in a fast paced environment.
  • Completes assignments within specified time lines.
  • Able to work well in a team environment on special projects.
  • Skill in working independently, but with supervision.
  • Prepares and maintains records and files that are organized, thorough and readily accessible.
  • Attend regular team meetings and report findings of projects.
  • When identifies a problem, informs supervisor in a timely manner so the problem can be corrected.
  • Transmits ideas and information in a clear and concise manner.
  • Assist supervisor with special duties/projects.
  • Other duties as directed by management.
