We are seeking a passionate Admission / Discharge Coordinator to join our VA REACH team, supporting our homes in Culpeper and Woodbridge, VA. 5,000 sign-on and retention bonus for FT, new hires! Your Role in Our Mission As the Admission / Discharge Coordinator, you will provide admission and discharge support to our Youth Crisis Therapeutic Home, Adult Crisis Therapeutic Home, and Adult Transitional Home. You will participate in the Admission Advisory Committee for our Adult Transitional Home and serve as a liaison with community stakeholders. You will be responsible for all discharge planning - including support team meetings, community appointments with the goal of transition, and overall discharge communication. In addition, this position maintains individual support plans and participates in the residential on-call rotation. Why Join Us?
