About The Position
The coordinator of DC planning is responsible for designing and coordinating comprehensive discharge plans in accordance with regulatory and patient satisfaction standards. Collaborates with nurse case managers and social workers regarding overall discharge planning needs.
Requirements
- 1 year experience with referral coordination
- Graduate of a licensed practical nurse program or Graduate of an accredited Physical Therapy Assistant Program or Bachelors of Social Work Degree
- PA Physical Therapist Asst Lic Required or PA Practical Nurse License Required
Nice To Haves
- 2 years experience in a Hospital Clinical setting
