About The Position

Leads and manages change through proactive educational offerings, project management, and on-going evaluation of clinical/business needs to strengthen staff competencies in the care and transition of patients throughout their continuum of care. Responsibilities include but are not limited to development, implementation and coordination of educational initiatives for case management team, project leadership serving as a subject matter expert to improve workflow, processes, and standardization where appropriate. Achieves educational and/or project objectives working in close collaboration with Chief Medical Officer, System Clinical Improvement, physicians, and other key stakeholders. Performs other duties as assigned.

Requirements

  • Three years of experience in Case Management.
  • Excellent interpersonal, analytical, and organizational skills.
  • Knowledge of applicable regulatory guidelines and regulations.
  • RN
  • DRIVER'S LICENSE (CURRENT)

Nice To Haves

  • Knowledge of computer software applications; product knowledge preferred.
  • CCM

Responsibilities

  • Develops and implements education programs, training and/or customized offerings to meet specific educational goals/objectives using a variety of educational platforms and presentation approaches.
  • Serves in key role as a Subject Matter Expert relative to case management and/or transition of care integration, process improvement and business/clinical solutions to include but not limited to Epic/BOC and Allscripts applications/software.
  • Completes assigned goals.
