More about our team St. Mary's Inpatient Rehabilitation offers transitional inpatient therapy and education for those with physical disabilities resulting from injury or illness. Our CARF-accredited center helps patients maximize function and independence while preparing them for their return home. The Inpatient Rehabilitation is a team of healthcare professionals working together to serve people with functional disabilities, their families, and the community, along with St Mary's Health Care System. We are helping our patients reach their full potential for independent living through education, therapeutic activities, and quality medical care. How you'll contribute An Admissions Coordinator who excels in this role: Interviews and assesses Elders and/or Elder's family, caregivers, and/or legal representatives. Determines, prioritizes, provides and/or arranges for needed internal and external services/interventions. Participates in case reviews to evaluate case management and progress. Consults with healthcare team members to promote, monitor, and evaluate compliance with patient's treatment plan. Performs Medicaid screenings as indicated for NHP. Incorporates the Eden Philosophy of living into every action undertaken throughout one's employment. Ensures each Elder is encouraged to reach his/her maximum functional ability and helps maintain these abilities. Committed to the development of the staff, Elders, and families as collaborative team members. Teamwork is a prime responsibility for each individual. Participate in the admission process of prospective residents in terms of their nursing needs and appropriate placement. Determine potential expense associated with a potential admission. Review all applications for admission, maintain waitlist, screen referrals, act as a resource to the community. Complete and assure the accuracy of the MDS for all residents. Maintain current working knowledge of Medicare criteria, serving as a resource for nursing staff and communicate changes in regulations. Maintain current working knowledge of MDS changes and most updated RAI manual. Attends and participates in weekly IDT and QAPI meetings. Actively involved in Family Care Conferences.
