The Coordinator - ACT Team (Assertive Community Treatment) is a full-time position responsible for overseeing the ACT program. This role involves coordinating care for consumers with psychiatric and medical issues, emphasizing the importance of both in recovery. The coordinator will work with healthcare and community agencies to manage the consumer's continuum of care, focusing on both medical and psychiatric needs. They will also educate staff and consumers about medications and medical conditions, assess consumers to differentiate between psychiatric and medical concerns, and possess in-depth knowledge of medications and physiological responses to illnesses and treatments. Superior psychotherapeutic skills, effective collaboration, understanding of training techniques for the SMI population, excellent communication, organizational, and prioritization skills are essential. The role requires managing and scheduling staff to ensure program success and services to ACT consumers, with an understanding of cultural diversity.
