About The Position

Bluestone delivers exceptional care to patients with complex, chronic conditions and disabilities through a unique, robust model. Multidisciplinary care teams collaborate with patients, families, and other healthcare providers to deliver preventative, proactive, and tailored care. Using an evidence-based approach, Bluestone care teams manage chronic conditions, address social determinants of health, manage transitions, and provide behavioral health support. This model has resulted in significantly fewer ER visits, hospitalizations, and readmissions. Care teams travel directly to patients in Assisted Living, Memory Care, and Group Home communities across Minnesota, Wisconsin, and Florida, supported by remote or corporate office staff. As a Contract Care Coordinator Assessor, you will work from home and travel approximately 30% of the time to meet with members in Hennepin County, MN. This position is available for part-time or full-time contractors with flexible work within regular business hours. The assignment is expected to last from ASAP through September/October 2026, with potential for Temp to Full-time opportunities.

Requirements

  • MN Licensed Social Worker OR Current Minnesota Registered Nurse license
  • Must have a valid driver’s license
  • Ability to work independently
  • Access to a private and compliant home office space
  • Creative problem-solving skills
  • Appreciation for working with diverse populations
  • Proven ability to communicate effectively with strong verbal skills
  • Excellent interpersonal and customer service skills
  • Demonstrated compatibility with Bluestone’s mission and operating philosophies
  • Demonstrated ability to read, write, speak, and understand the English language

Nice To Haves

  • MNChoices experience highly preferred (paid training available)
  • One or more years of experience working with the geriatric or certified disabled population in case management/care coordination, Home Care, Group Homes, Nursing Homes, TCU or Assisted Living settings preferred

Responsibilities

  • Contacting new enrollees to complete the initial assessment
  • Contractors working full time hours will complete approximately 8 assessments per week
  • Reviewing current health needs, identifying goals, and developing individualized care plans and transitions of care
  • Helping connect members with resources and services
  • Completing required documentation
