About The Position

CC CI Improvement Specialists are responsible for performing various key functions within the Integrated Northern Health portfolio. The Specialist will lead and sustain improvements in purposeful rounding, restraint management, falls prevention and injury reduction and other Continuing Care (CC) priorities. Works collaboratively with the interdisciplinary team to ensure that the best leading and evidence-based practices are used to provide high quality, safe care. Provides on-site education to nursing teams to build capacity and create a positive, supportive care environment for residents and staff. Collaborates on development of policy and procedure for CC. Seeks out best, leading and evidenced based practices for implementation within the care team. Role model culture change and resident directed care within CC. Conduct audits to evaluate nursing department effectiveness and compliance with standards, regulations, policy and procedures; provides recommendations for corrective actions; works with the nursing team to address areas of concern.

Requirements

  • 3-5 years experience in Long Term Care, Rehabilitation or Geriatric Services
  • Undergraduate degree or diploma in Nursing
  • Valid Class 5 driver's license
  • Licensed, and in good standing, with regulatory body, if applicable
  • Undergraduate degree or an equivalent combination of education and experience
