The Continuing Care Coordinator is responsible for the coordination of continuing recovery services for clients returning to the community after completion of the Clinical Stabilization Services program and for providing aftercare counseling services to clients on campus. This role involves performing case management activities based on clients' continuing care plans regarding initial housing, job searches, and establishment of medical services, social services, education, support services, and services from other community providers. The coordinator will also develop positive relationships with community providers to coordinate continuing care recovery referrals and schedule initial appointments. A key aspect of this role is working closely with the Clinical staff in the Clinical Stabilization Services program to ensure smooth client transitions from residential treatment to the continuing care program, and collecting data on program success rates.
