About The Position

Charlie Health is seeking a Continuing Care Coordinator to manage client-related communication and coordination with referral sources such as hospitals, outpatient practices, schools, and governmental organizations. This role involves providing treatment updates to referring providers and planning discharge for clients in our care. The goal is to connect individuals with life-saving behavioral health treatment through personalized, virtual care rooted in connection. As a rapidly growing organization, Charlie Health is redefining behavioral health treatment and expanding access to meaningful care.

Requirements

  • Bachelor’s degree in health sciences, communications, psychology, social work, or related field.
  • Minimum of 2 years of relevant work experience (e.g., experience in healthcare, preferably in customer / patient-facing roles such as case management, discharge planning, referral relations, admissions, or outreach).
  • Strong interpersonal, relationship-building and listening skills.
  • Metrics- and results-oriented mindset, with experience working against concrete targets.
  • Excellent written and verbal communication skills.
  • Extreme organization and attention to detail.
  • Work authorized in the United States and native or bilingual English proficiency.
  • Ability to thrive in a fast-paced environment and learn quickly.
  • Proficient in Salesforce and Google Suite/MS Office.

Responsibilities

  • Providing consistent, high-quality treatment updates to referral sources and answering their questions about referred clients.
  • Building trusting relationships with referral sources through professional and collaborative communication.
  • Developing comprehensive and clinically appropriate discharge plans for clients completing treatment.
  • Identifying appropriate aftercare resources and engaging with clients to facilitate a smooth transition.
  • Ensuring a supportive, positive experience for clients and referral sources/external providers.
  • Working directly with clients, families, and referral sources to understand their needs and preferences.
  • Making accurate and timely referrals to aftercare resources for clients discharging from Charlie Health.
  • Using and maintaining a nationwide provider database to identify appropriate referral options.
  • Making referrals using external provider’s preferred communication channels.
  • Following up with clients and/or external providers to verify placement.
  • Communicating with referral providers about new referrals and ensuring they have all necessary information and clinical materials.
  • Documenting case management contacts in progress notes and communicating with therapists/case managers and treatment team about contact and updates on discharge planning status.
  • Collaborating closely with internal stakeholders at Charlie Health (e.g., clinical team, admissions team).
  • Working closely with the Clinical Outreach and Partnerships teams to build a deep understanding of referral sources and their services.
  • Adhering to stated policies and procedures and achieving performance metrics goals.
