The purpose of this position is to coordinate, implement, and evaluate community-based chronic disease prevention and management initiatives that improve the overall health and wellness of community members. The Chronic Disease Prevention, Wellness, and Screening Coordinator will provide leadership for chronic disease education, lifestyle coaching, fitness and wellness promotion, community health screenings, and care coordination activities designed to reduce risk factors and improve health outcomes. This position serves as a bridge between community members, healthcare providers, public health programs, and community resources to increase access to preventive services, support healthy lifestyle changes, and promote early detection of chronic diseases. The Coordinator will lead efforts related to diabetes prevention and management, cardiovascular health, obesity prevention, nutrition, physical activity, tobacco cessation, healthy aging, and other chronic disease priorities. The position will also oversee community health screenings, referral processes, wellness activities, and population health initiatives that align with the Winnebago Public Health Department's strategic goals and community health improvement priorities.
