About The Position

Actively Hiring Concierge Registered Nurses for La Jolla & North San Diego County, CA. This role offers a pay rate of $45.00 – $80.00 per hour, depending on experience, for 12-hour day shifts on a per diem basis. The position is for a dedicated Registered Nurse (RN) who enjoys spending quality, one-on-one time with older adults and delivering premium, high-quality care in their homes. In this concierge role, you will collaborate closely with an interdisciplinary team to develop, implement, and manage individualized care plans while providing continuous support and feedback to clients, their families, and nursing peers.

Requirements

Current, unrestricted California registered nurse license
3 years of experience as a registered nurse
Home Health experience preferred
Proof of Negative TB result
Physical Screening Documentation within the last year
Active California Drivers license and car insurance
Cell phone with Internet Access

Responsibilities

Conduct thorough clinical assessments of clients' physical, emotional, and environmental needs.
Develop customized, evidence-based care plans and establish measurable health and wellness goals for each client.
Oversee the ongoing management of chronic conditions, ensuring proactive and preventative interventions.
Provide specialized skilled nursing interventions, including post-operative care, wound care, and medication management as needed.
Serve as a trusted resource by educating clients and their representatives on diagnoses, treatment plans, and self-care strategies.
Deliver comprehensive wellness support that promotes independence, safety, and an enhanced quality of life for seniors.
