About The Position

An established, physician-led concierge medical practice is seeking a Primary Care Physician to join its collaborative, relationship-focused care model. This practice emphasizes high-touch, personalized, and longitudinal care, allowing physicians to build meaningful relationships with patients while delivering evidence-based medicine. Physicians work within a highly coordinated, team-based environment, partnering with clinical staff and specialists to proactively manage patient health and well-being.

Requirements

• MD or DO from an accredited medical school
• Residency completed in Internal Medicine, Family Medicine, or Medicine-Pediatrics
• Minimum 5 years of clinical practice experience
• Strong interpersonal and communication skills
• Comfortable with EHR systems and digital tools
• Ability to work effectively in a team-based care model
• Strong organizational and multitasking abilities

• Background in longitudinal primary care preferred

Responsibilities

• Build and maintain long-term patient relationships
• Diagnose and manage acute and chronic conditions
• Conduct annual wellness visits, routine check-ups, and home visits as needed
• Provide after-hours support via phone for patient care
• Monitor physical and mental health development
• Lead and collaborate with a dedicated care team
• Develop personalized care plans and health reports for patients
