About The Position

The Concierge/Community Coordinator plays a central role in delivering personalized, person-centered care and a exceptional community experience to residents. You'll work directly from each resident's Individual Service Plan (ISP), ensuring consistent, compassionate care while providing leadership and guidance to Caregivers and Med Techs on your team. This role reports to the Health and Wellness Director.

Requirements

• High school diploma or GED required
• Minimum six months of experience in a caregiving or customer-facing hospitality role, preferably with seniors
• Ability to read, write, and comprehend English
• Must be at least 18 years of age
• Must pass a criminal history check
• Reliable transportation required
• Genuine interest in and compassion for working with seniors
• Strong interpersonal skills with a warm, professional presence
• Emotional maturity to handle sensitive situations with grace
• Ability to work within a multidisciplinary team setting

Nice To Haves

• CNA certification is a plus but not required

Responsibilities

• Oversee and implement each resident's Individual Service Plan, ensuring person-centered care is delivered consistently across the team
• Monitor task completion in the EMR system weekly and track changes in resident needs, reporting updates to the Director of Resident Care and Executive Director as needed
• Maintain shift-to-shift communication logs and supporting documentation
• Assist residents with activities of daily living (ADLs) as needed, including personal hygiene, dressing, ambulation, transfers, toileting, nutritional intake, and medication reminders
• Ensure bath lists, laundry lists, daily task lists, and other care communication systems reflect each resident's current needs
• Train, supervise, and motivate Resident Care Partners and other care team members
• Conduct at least monthly verbal reviews of resident needs to identify undocumented changes and compare ISP documentation to actual care delivered
• Serve as a welcoming point of contact for residents, families, and visitors, reflecting the community's commitment to warmth and hospitality
