The Complex Inpatient Case Manager at Wake Forest Baptist Medical Center is responsible for performing thorough patient assessments, identifying critical health issues, and implementing targeted interventions and patient-family centered care plans to achieve optimal health outcomes. This role involves collaborating and negotiating effectively with clinically complex patients, families, and the clinical team to achieve patient and organizational goals regarding care needs, choices, and satisfaction during discharge planning and care transitions. The Case Manager will aggregate, analyze, interpret, and report data on patient outcomes and resource utilization, facilitate reporting of utilization monitoring and review activities, and provide continuity of care and discharge planning services compliant with regulatory standards. They will offer coordinated, relevant options and services based on assessed needs, participate in communication processes for smooth transitions, and provide advanced clinical guidance and mentorship to frontline care management team members. The role also includes leading initiatives to enhance care quality, patient safety, and healthcare delivery efficiency, advocating for patients and families, and initiating internal and external referrals. Documentation of discharge planning interventions and utilization review activity is crucial, as are concurrent and retrospective reviews. Effective communication with the healthcare team, partnership with Social Work and unlicensed support personnel, and leadership in multidisciplinary rounds are key. The Case Manager will collaborate with managers, physicians, and treatment teams on physician practices and patient care plans, refer cases to physician advisors as needed, and maintain knowledge of healthcare regulations, reimbursement issues, and community resources. They will deliver CMS regulatory notices within established timeframes and develop relationships with community-based agencies. The role requires representing Advocate Health positively and collaborating with Advocate Health Ambulatory Care Management and Continuing Health. A strong understanding of age-specific care needs is essential.
