About The Position

The Care Manager is a licensed health care professional who is responsible for both utilization review and discharge planning. The Care Manager collaborates with physicians, nurses, social workers, and other members of the health-care team. The Care Manager maintains practice patterns consistent with state licensure and Joint Commission Standards. The Care Manager adheres to professional care management ethical standards of practice. Requirements

  • Current Massachusetts Nursing License
  • Graduate of an accredited school of nursing, RN or BSN
  • Experience in discharge planning and utilization review

Responsibilities

  • Demonstrates a comprehensive knowledge of case management theory and practices.
  • Demonstrates ability to effectively plan for patients' continuity of care needs in ambulatory care and emergency care settings.
  • Clinical documentation in Medhost and Meditech
  • Demonstrates understanding of the principles of Care Management
  • Demonstrates accurate understanding of InterQual Criteria
  • Demonstrates effective communication in a timely fashion with all involved departments concerning the appropriateness of bed status and insurance authorization
  • Demonstrates effective communication with health plan case managers regarding the policies for the ECC patient.
  • Demonstrates knowledge of general nursing theory and practices
  • Demonstrates knowledge of discharge planning rules and regulations
  • Informs director of any and all discharge planning incidents which are barriers to effective.
  • Complies with established departmental policies, procedures and objectives.
  • Attends variety of meetings, conferences, seminars as required or directed.
  • Demonstrates use of Quality Improvement in daily operations.
  • Complies with all health and safety regulations and requirements.
  • Respects diverse views and approaches, demonstrates Standards of Respect, and contributes to creating and maintaining an environment of professionalism, tolerance, civility and acceptance toward all employees, patients and visitors.
  • Maintains, regular, reliable, and predictable attendance.
  • Performs other similar and related duties as required or directed.
