Crossroads Turning Points (CTP) is seeking a dynamic individual to serve as the Complex Case Manager for Level 1 or 2 for the Lead Program. The Law Enforcement Assisted Diversion (LEAD) program is a harm reduction initiative focused on connecting individuals with low-level substance use offenses to social services, aiming to reduce their future involvement with the criminal justice system and enhance their quality of life. The Complex Case Manager (CCM) works directly with individuals facing challenges such as substance use disorder (SUD), mental health issues, and homelessness, providing them with stabilizing support and services. This role involves meeting with LEAD participants and community referrals, conducting intakes and assessments, and facilitating connections to available resources. The CCM is also responsible for developing and maintaining collaborative relationships with housing providers, service providers, law enforcement, and other community organizations.
