About The Position

Galileo is seeking a culturally-competent Nurse Practitioner to join their Population Health team. This role involves providing high-quality, comprehensive follow-up care to patients post-hospital discharge using phone and video-based care. The Nurse Practitioner will cover the Transitions of Care program, conducting telephonic outreach and scheduled video/phone visits. Key responsibilities include medication reconciliation, symptom screening, ordering follow-up labs/medications/supplies, building patient trust, explaining Galileo's care model, connecting patients to services, and coordinating care across various settings (hospital, ED, pharmacies, community providers). The role also involves collaboration with the nursing team and accountability for individual performance targets while contributing to team-level population health goals.

Requirements

  • Certified Nurse Practitioner in Adult, Family or Geriatrics
  • Active state licensure in SC, with the ability to obtain additional state licensures
  • 5+ years of post-graduate clinical experience in Primary Care or Geriatrics
  • Direct experience engaging Medicare and Medicaid populations managing complex, comorbid conditions
  • Motivational interviewing skills
  • Comfort in a digital-first environment, with experience in or a genuine appetite for virtual care delivery
  • Solid tech proficiency
  • Excellent written and verbal communication skills
  • Available to work 32-36 hours/week

Nice To Haves

  • Bonus if you have active licenses in TX, NJ, PA, IL, NY, MA, MN, and/or LA
  • 2 of the 5 years of clinical experience may be substituted by time spent working at a Skilled-Nursing facility or Long Term Care facility
  • Familiarity with Google Suite and Slack is a plus
  • Bilingual in English and Spanish, including cultural fluency, is a strong bonus

Responsibilities

  • Cover care needs for the Transitions of Care program.
  • Conduct telephonic outreach (“cold calls”) and scheduled video/phone visits for recently discharged patients.
  • Perform medication reconciliation.
  • Conduct symptom screening.
  • Order any needed follow-up labs, medications, or supplies.
  • Navigate conversations with patients to build trust, explain Galileo's care model, and connect them to services.
  • Coordinate care across settings, including hospital and ED transitions, pharmacies, and community providers.
  • Collaborate with the nursing team.
  • Be accountable to individual performance targets while contributing to team-level population health goals.
