About The Position

Reporting to the System Executive Director of Case Management, the Complex Care Coordinator is responsible for overseeing the coordination of healthcare services for patients in the MUSC system with complex care needs, including but not limited to SCDC (South Carolina Detention Center), Behavioral Health, and VA, as well as other long stay patients. The Complex Care Coordinator will work to build trusting relationships and ensure that patient movement for unique patient populations is a seamless process performed in a safe, timely, and appropriate manner. This includes managing transportation logistics for SCDC patients, patient admissions and discharges, clinical updates, and coordination with various outside agencies and MUSC legal counsel. The Complex Care Coordinator is the primary contact for specialty populations within the MUSC Health system, working within the operational requirements to ensure patients' safe and timely movement. The role requires effective communication, prioritization of services, and collaboration with various healthcare providers, within and outside of the MUSC network. Remote position with travel as required.

Requirements

  • Bachelor's degree in Nursing required.
  • Licensure by the South Carolina Board of Nursing.
  • OR Master’s degree in Social Work from an accredited school of social work.
  • Proficient in Microsoft Suite Solutions
  • Effective oral, written, and interpersonal communication skills
  • Ability to Multitask
  • Licensure as a registered nurse or social worker by the state of South Carolina or compact state required.
  • Current American Heart Association (AHA) Basic Life Support (BLS) certification or American Red Cross BLS for Healthcare Providers certification is required.

Nice To Haves

  • 3 years of case management work experience preferred.
  • Certification in Case Management preferred.
