The Community Support Worker is part of the Community Supports & Outreach Team (CSO) within the Supportive Housing Department at WoodGreen Community Services. This role supports adults 18+ with mental health and/or substance use issues, concurrent disorders, and dual diagnosis. The program offers case management, recreational programming, and hoarding and pest supports within anti‑oppressive, trauma‑informed, harm reduction, and mental health recovery frameworks. Clients live across four WoodGreen-owned buildings and 36 privately owned units throughout Toronto, and the role includes community-based work and travel during work hours. The Community Support Worker reports to the Manager of Supportive Housing Programs. This position follows WoodGreen policies, procedures, and all relevant legislation, while supporting our commitment to community development, anti‑discrimination, and promoting wellness and self‑sufficiency. Due to WoodGreen’s multi‑site operations, staff may be relocated to another site performing the same duties based on program needs, with reasonable notice provided. The Community Support Worker uses a client‑centered, trauma‑informed approach to assess tenant issues, respond to needs, de‑escalate crises, and provide individualized case management, counselling, and service planning. The role collaborates closely with supervisors, managers, housing staff, and external service providers to support tenant wellness, prevent evictions, mediate disputes, and strengthen community connections. The approach is inclusive, respectful, collaborative, recovery‑focused, and grounded in harm reduction.
