About The Position

The Community-Based Support Specialist provides direct support services to children, youth, and families through Aftercare, Behavioral Health Intervention Services (BHIS), and other community-based programs as assigned. This position focuses on helping clients develop skills, improve functioning, access resources, and achieve individualized treatment and service goals. Services are delivered in homes, schools, and community settings and are designed to promote family stability, positive behavioral health outcomes, independence, and long-term success. The specialist works collaboratively with clients, families, schools, healthcare providers, and community partners to ensure coordinated care and support.

Requirements

• Must be at least 21 years of age.
• Ability to work independently and effectively manage a flexible schedule.
• Strong verbal, written, organizational, and interpersonal communication skills.
• Ability to establish and maintain positive working relationships with clients, families, and community partners.
• Reliable transportation that meets agency requirements.
• Valid driver's license with an acceptable driving record.
• Proof of insurance meeting agency standards.
• Ability to travel throughout assigned service areas as required.

Nice To Haves

• Bachelor's degree in Social Work, Human Services, Psychology, Education, Counseling, or a related field preferred.
• Relevant lived experience and/or equivalent experience may be considered in lieu of educational requirements, where applicable.
• Minimum of one year of experience working in human services, behavioral health, child welfare, education, or a related field preferred.
• Experience working with children, adolescents, and families is preferred.
• Knowledge of behavioral health, trauma-informed care, crisis intervention, and community resources is beneficial.
• 10% bilingual stipend available.

Responsibilities

• Provide community, school, and home-based skill development, behavioral support, education, and intervention services to children, youth, and families.
• Assist clients in developing coping skills, communication skills, social skills, daily living skills, emotional regulation, and problem-solving abilities.
• Support successful transitions for youth and families through Aftercare services and other assigned community-based programs.
• Complete assessments, service plans, progress notes, discharge summaries, and other required documentation in accordance with program, funding source, and agency requirements.
• Maintain timely and accurate records within agency systems.
• Collaborate with therapists, case managers, schools, healthcare providers, and other community partners to coordinate care and advocate for client needs.
• Provide referrals and assist families with accessing community resources and informal support systems.
• Participate in treatment team meetings and staffing as required.
• Collect program data and participate in quality improvement activities.
• Maintain productivity standards and service delivery requirements established by the agency and funding sources.
• Travel to homes, schools, community locations, and other service settings as required.
• Participate in supervision, consultation, and team meetings.
• Complete required agency, state, and program-specific training.
• Demonstrate flexibility in scheduling to meet client and program needs.
• Maintain professional boundaries and confidentiality in accordance with agency policy and HIPAA regulations.
• Foster a positive team environment and collaborative working relationships.
• Represent the agency professionally with clients, families, referral sources, and community stakeholders.
