About The Position

The Assertive Community Treatment Team (ACTT) serves adults 18 years of age and older, providing intensive community-based treatment, support, and rehabilitation to individuals with a serious persistent mental illness (SPMI) or a dual diagnosis of SPMI and a Substance Use Disorder, who require a high level of service needs. The Community Support Specialist II partners with individuals to achieve their recovery goals, conducts Co-occurring Screens, C-SSRS, and Functional Assessments. This role involves collaborating with clients to create individualized, person-centered treatment plans and documenting their progress. Specialists use Motivational Interventions to help clients meet their goals and progress in recovery, utilizing stage-wise interventions. They provide psycho-educational rehabilitation services through group and individual modalities, support individuals in connecting with natural and community supports, and develop crisis and safety plans. The position also includes teaching essential life skills such as obtaining/maintaining housing and utilities, using transportation, meal planning, engaging with family and friends, ensuring community safety, caring for children, and problem-solving daily issues. Participation in family education and support activities is also expected.

Requirements

  • Bachelor’s degree or equivalent education and/or work experience required.
  • Valid driver’s license required

Responsibilities

  • Partnering with the individual to accomplish their recovery goals.
  • Conducts Co-occurring Screens, C-SSRS and Functional Assessments.
  • Collaborates with individuals to create individualized, person-centered treatment plans and documents clients progress during treatment.
  • Use Motivational Interventions to assist clients in meeting their goals and moving along their paths to recovery, utilizing stage wise interventions.
  • Provide psycho-educational rehabilitation services via group and individual modalities.
  • Support individual with linking and engaging with natural (family, friends) or community supports.
  • Develop crisis plans and Safety Plans for clients and work with other staff to respond to crisis situations as needed.
  • Provide skills teaching, such as assisting clients in obtaining/ maintaining housing and utilities; learning how to use transportation services, planning and preparing nutritional meals, engaging and communicating with family and friends, safety in the community, caring for children and other family members, and solve everyday problems that arise. Participate in family education and support activities.
  • Documents client progress per agency policies.
  • Attend and participate in staff meetings, trainings, case conference, supervision and other meetings as assigned.
  • May perform other assignments and duties, as assigned.
