About The Position

The Community Support Specialist (CSS) works under the direction of the Program Manager to provide case management and supportive services to participants in the housing program. The program's goal is to help participants access needed services, such as referrals, assistance with applying for benefits/income, and other resources to support them in achieving permanent housing and ending their homelessness. The CSS works independently but is expected to consult with the Program Manager as needed. Other duties include community building, consistent case noting, file upkeep, staff meeting participation, assistance to larger housing programs, and other administrative, financial, and programmatic duties. CHS utilizes the Housing First and Harm Reduction service delivery models, prioritizing housing without requiring abstinence from harmful behaviors.

Requirements

  • A valid PA driver’s license and acceptable driving record.
  • Ability to drive to various locations within Allegheny County.
  • Excellent communication and interpersonal skills.
  • Exceptional customer service communication.
  • Cultural competence.
  • Experience and ability to de-escalate mental and behavioral health crisis situations.
  • Strong organizational, phone, and computer skills.
  • Working knowledge of the social service system.
  • Willingness to work independently and collaboratively.
  • Strong ability to navigate the Internet to access resources.
  • Ability to work and travel independently.
  • A high school diploma or equivalent, along with a combination of work, education, and life experiences as approved by the agency and funder.
  • Act 34 Criminal clearance.
  • Act 33 Child Abuse clearance.
  • FBI Criminal clearance.
  • Reliable transportation.

Responsibilities

  • Provide individual assessment of participant needs.
  • Develop a professional and supportive relationship with participants while maintaining appropriate boundaries.
  • Facilitate locating and securing safe and affordable housing for participants and build landlord connections.
  • Assist participants in developing greater independent living skills and provide tenancy education.
  • Conduct home visits with participants to ensure housing safety and provide case management.
  • Provide referral services to appropriate programs (D&A, medical, MH, housing, vocational, educational, family services).
  • Advocate for appropriate benefits and services with external providers.
  • Support participants in connecting with and utilizing community resources.
  • Create goal plans with participants and assist them in meeting goals related to income, housing, and supportive services.
  • Submit check requests for participants' rental subsidies.
  • Complete case notes, files, and maintain accurate, timely documentation.
  • Attend staff meetings and training courses.
  • Communicate with CHS staff, other service providers, and landlord/building management regarding participant issues.
  • Maintain open communication and cooperate with other CHS staff to achieve agency and program goals.
  • Support harm reduction, trauma-informed care, person-centered, and housing first principles.
