About The Position

The Community Support Provider (CSP) provides client advocacy, ensures continuity of care, supports clients in time of crisis, provides/procures skills training, ensures the acquisition of necessary resources, and assists the clients in achieving community/social integration.

Requirements

  • Must be at least 21 years of age.
  • Successful completion of background checks/references including a valid driver’s license. The driving record must be in accordance with the agency’s guidelines.
  • If in lived substance abuse recovery, two years of continuous sobriety/ “clean time” is required and maintained throughout employment.
  • Willing to obtain a flu shot annually (except for those allergic to the vaccine or who have a religious exception) or must wear a mask when agency deems appropriate due to times of high illness.
  • Must comply with agency policies and procedures, including those regarding conduct, confidentiality, and record keeping.
  • Two years of coursework in a human services field, or two years of experience or training in a human service field and two years of lived recovery experience with demonstrated skills in treatment of individuals with a behavioral health diagnosis required.
  • Must have basic computer knowledge with the ability to navigate email, spreadsheets, E.H.R systems, phone systems, printers/copier/scanner etc.
  • Has the ability to apply critical thinking skills by using logic and reasoning to identify the strengths and weaknesses of alternative solutions, conclusions, or approaches to problems.
  • Possesses knowledge of case management functions, signs and symptoms of mental illness, and the physiological impact of mental illness.
  • Is familiar with the community/state services/resources available to persons with chronic mental illness and/or substance abuse/dependence.
  • Can work independently, organize work, and determine priorities to meet job expectations and deadlines.

  • Bachelor’s Degree or higher in psychology, sociology or a related human service field is preferred.

Responsibilities

  • Maintains a case/production load as defined by Health and Human Services service definitions (1:25) and meets and/or exceeds productivity expectations set forth by the agency.
  • Conducts comprehensive, strengths-based assessment with each new client entering CSP-MH or CSP-SA services.
  • Collects information to develop individual Service Plans with each client on an on-going basis.
  • Directly provides active rehabilitation and support interventions with focus on activities of daily living, education, budgeting, medication compliance and self-administration, relapse prevention, social skills, and independent living skills.
  • Coordinates with rehabilitation/treatment team/primary supervising practitioner on progress in the areas of medication compliance, relapse prevention, social skill acquisition and application, education, and substance use/abuse.
  • Assists in the coordination of services and case management activities including coordination or assistance in accessing medical, social, education, housing, transportation, or other appropriate support services as well as linkage to more/less intensive community services.
  • Provides support and intervention in times of crisis. Crisis relapse intervention and involvement to transition consumer’s return to the community and avoid the need for higher levels of care. May assist in crisis/relapse prevention planning.
  • Visits and/or contacts with the clients on the caseload are consistent with the service plans, client and/or family needs.
  • Encourages follow-through by clients on hospital discharge recommendations.
  • May arrange for and/or provide transportation for clients to and from service appointments.
  • Advocates for their clients’ rights and assists clients in ensuring that their rights are protected and preserved.
  • Registers and authorizes DBH/CDS-eligible clients at the time of admission; authorizes clients for CSP services, as required by other pay sources. Re-authorizes clients for CSP services when authorizations expire.
  • Documents all client services/contacts in the client’s clinical record and completes necessary clinical documents and evaluations within timeframes given. All assigned client charts are kept current and accurate as defined by agency standards.
  • Completes all steps to discharge a client completely after services have ended.
  • May provide 24-hour emergency coverage duties for the Community Support Program, in-office, or after-hours, as assigned.
  • Maintains communication with service agencies to monitor follow-through by clients.
  • Communicates regularly with supervisor and other team members regarding assigned clients and areas of concern.
  • Participates in staffing, case conferences, and other meetings held by providers or agencies involving the client.
  • Participates in quality assurance and evaluation processes of the program.
