About The Position

Looking for fun, creative and flexible staff to provide community based support services for individuals with developmental disabilities of all ages. Community Support Professionals work one on one with individuals on their caseload to implement unique services, to explore and build skills which support increased independence, inclusion, and community engagement. This position can be part time or full time in the Tompkins County area, with 6-8 hours preferred on Saturdays. This position requires that you have your own transportation.

Requirements

  • High school diploma or GED
  • Ability to meet the agency's approved driver criteria (3 years’ experience with a driver’s license and no more than 4 points)

  • Ownership/usage of personal smartphone is strongly preferred

Responsibilities

  • Responsible for the health and safety of the individuals supported
  • Provide individualized services to people with disabilities and their families which may include: respite for families, teaching, modeling, and supporting skill development in a variety of areas at home and in the community
  • Participate as an active member of the person’s team providing ideas, input, and advocacy for the person related to his or her Staff Action Plan
  • Follow objectives as described in the person’s Staff Action Plan and as discussed with supervisor
  • Attend training to improve services provided
  • Complete job requirements (training, PPD, etc) within appropriate time frames
  • Report any potential incident to the designated Family Support Coordinator/Supervisor immediately, or the day that it occurred.
  • Accurately enter time and services into Racker’s electronic health record, eVero, documenting service times, supports provided, and mileage according to program and regulatory requirements (e.g., contemporaneously documenting service provision)
  • Use of smartphone for documentation purposes required. Ownership/usage of personal smartphone is strongly preferred
  • Communicate regularly with supervisor regarding service provision
  • Attend scheduled supervisory meetings regularly
  • Uphold standards of professionalism with regard to behavior and dress
  • Assume responsibility for related duties as required or assigned
