The Community Support Professional provides direct prevention support to individuals diagnosed with behavioral health diagnoses served by the REACH program. Also assist with the development of and staff training in individualized Crisis Education and Prevention Plans. Works as part of the REACH team to ensure services and resources are coordinated among providers as needed to help individuals served live and participate optimally in the community. Full-Time and Part-Time positions are available, with on-call as optional!
