This role involves supporting individuals with disabilities to become valued members of their communities, enhancing their inclusion, and providing direct training and support in their homes and communities. The professional will empower individuals and their families by providing necessary support and information for informed choices, helping to build self-esteem and assertiveness for self-determined lives. This includes learning about individual needs and interests, identifying community supports, and assisting in developing and sustaining relationships. The role also involves managing daily life aspects using habilitative strategies, maximizing choices, pursuing professional growth, and facilitating self-advocacy. Crisis prevention, intervention, and follow-up are key, as is contributing to the organization's mission and completing documentation efficiently while ensuring compliance with OPWDD regulations and agency policies. Facilitating communication with employees and outside agencies for community opportunities and participating in meetings are also part of the role.
