The Together in the Community Program provides community-based support services for adults with autism and developmental disabilities, helping build independence, daily living skills, and meaningful community involvement. We partner with local school districts to support transition-aged students through life-skills development and real-world community experiences that prepare them for greater independence and future employment opportunities. As defined by an individual’s Individual Service Plan, develop, provide one-to-one interventions to an adult consumer at home, on the job site, or other community-based setting. Provide and submit required documentation of service provision for legal/reimbursement purposes. Input and produce documentation in Devereux’s electronic health record (EHR) system in accordance with Devereux’s procedures and as more fully outlined below in Exhibit B. Attend review meetings with Devereux representatives as mandated by federal, state and local laws as well as Devereux. Participate in and submit documentation of training as directed by Devereux and/or mandated by federal, state and/or local laws. Upon request, provide a current and valid driver’s license, current and valid vehicle registration and current and valid automobile insurance policy for the vehicle that will be utilized for rendering services under this Agreement. It is the responsibility of the contractor to secure and maintain current and valid vehicle inspection, registration, insurance and a driver’s license in order to transport consumers for activities pursuant to this Agreement. It is also the responsibility of the contractor to immediately cease transporting clients and to immediately notify the Director of the Program if the Contractor falls out of compliance with any of the requirements above. Contractor will not be reimbursed the cost of mileage.
