This position is for a Community Support Mentor within the Community Supported Living (CSL) program. The role involves empowering individuals with disabilities to achieve their work and life goals by supporting them to live independently in their community. Requirements

• Ability to support individuals to live independently in their community.
• Ability to work as a mentor and build relationships with individuals to assist them to connect to their community.
• Ability to support individuals with daily activities needed to live independently, including cleaning, cooking, laundry, medication support, bill paying, and personal care support.
• Ability to develop community relationships that fosters connections between the person supported and community organizations.
• Ability to make decisions and work independently.
• Requires a company email address which requires an application to be downloaded on your personal cell phone for multi-factor authentication (MFA) to ensure cyber security.
• Reliable vehicle (unless determined exempt with HR).

Responsibilities

• Support individuals to live independently in their community.
• Act as a mentor and build relationships with individuals to assist them in connecting to their community.
• Support individuals with daily living activities such as cleaning, cooking, laundry, medication support, bill paying, and personal care.
• Develop community relationships that foster connections between the person supported and community organizations (e.g., volunteering or joining a club).
• Make decisions and work independently.
• Utilize a company email address which requires downloading an application on a personal cell phone for multi-factor authentication (MFA) for cybersecurity.
• Possess a reliable vehicle (unless exempt by HR).
