About The Position
This is a Part-Time position at CHOICE Crystal Lake, located in Crystal Lake, IL. The role involves assisting in the development and implementation of programming to promote client independence and functional skill acquisition. The Community Support Coach will provide hands-on assistance to clients with daily living activities, including feeding and toileting, and support one individual in community outings and day program attendance. The position offers paid training.
Requirements
- High school diploma or equivalent
- Valid driver’s license required
- Must be 21 years of age
Responsibilities
- Assist in the development and implementation of programming that promotes client independence and acquisition of functional skills.
- Assist one individual served in community outings.
- Assist in developing appropriate curriculum for implementing client program objectives and employment opportunities.
- Provide hands-on assistance to clients with activities of daily living which may include feeding and toileting.
- Provide assistance to one individual who attends the day program.
