About The Position

The Community Services Direct Support Professional provides direct care by implementing Habilitation Plans. The role supports people receiving services in maintaining safety and comfort, building independent-living skills, pursuing individual goals, and participating in inclusive community activities. The position is part-time, up to 29 hours per week, and is based in Broome and Tioga County.

Requirements

High school diploma or GED.
Possess a valid driver’s license acceptable to the agency’s insurance carrier.

Nice To Haves

Two years of experience working with individuals with disabilities, or a combination of education and experience that, in the opinion of the reviewing agency, exceeds the program’s standards.

Responsibilities

Provide supervision and direct care needed to maintain the safety and comfort of people receiving services.
Transport or accompany people receiving services to community outings, appointments, or recreation activities to support community integration and exploration.
Develop and plan inclusive community activities.
Develop and maintain positive relationships with area businesses.
Assist people receiving services with daily or routine domestic and personal care tasks.
Foster and promote skills that support independent living.
Assist with developing Habilitation Plans and follow those plans when providing training and assistance.
Train and assist people receiving services to develop, maintain, or improve assigned individual goals, living skills, and personal needs.
Supervise people supported during program hours and community outings to ensure their health and safety.
Complete required documentation in accordance with applicable rules, regulations, policies, procedures, and billing practices.
