About The Position

The Community Schools Coordinator leads the implementation of the community school model at Tubman Elementary School, serving as a bridge between the school and the broader community, helping to integrate behavioral health, health, family-engagement, and social-service supports into the school environment. Working in close partnership with school leadership, staff, students, families, community-based organizations, and other stakeholders, the Coordinator identifies school and community needs, coordinates resources and partnerships, develops programming that supports student success and family well-being, and monitors progress toward program goals. Additional responsibilities include supporting the development and management of the budget, writing grant reports, and seeking additional funding sources to support sustainability, as needed. This position must also facilitate and participate in the Community School Advisory Board meetings.

Requirements

• Bachelor’s in Social Work, Health Education or other Human Services field.
• Must be able to work independently with minimum oversight and demonstrate attention to detail, accuracy, and quality awareness.
• Proven analytical and problem-solving skills.
• Proven effective written and interpersonal communication skills, well organized, ability to multitask and work independently, promote flexibility and teamwork.
• Provide excellent customer service to our external and internal customers.
• Working knowledge of Microsoft Office applications (Outlook Email, OneDrive, Teams, Word, Excel, PowerPoint); knowledge of patient confidentiality issues and HIPAA compliance/regulation are essential.
• Ability to communicate effectively in English is required.

Nice To Haves

• 1 - 3 years as case manager or clinical support specialist preferred but not required.
• The ability to communicate in Spanish is preferred but not required.

Responsibilities

• Develop and implement a needs assessment survey to use along with other school data to create a community school plan
• Work with school staff and partners to reduce barriers to attendance, participation, and academic success through school-wide and individual student support.
• Provides case management to connect students and families with school-based and community resources that address health, behavioral health, housing, food, employment, and other needs.
• Coordinate family-engagement activities that build authentic partnerships between families and the school.
• Links students and their families to Mary’s Center’s medical, dental, and mental health services
• Supports students/families in accessing care at Mary’s Center’s health center, or other provider as identified;
• Ensures that participant’s family understands protocols for attending appointments;
• Plan and coordinate programs such as after-school activities, workshops, family events, health and wellness initiatives, student enrichment opportunities, and resource fairs.
• Develop an annual calendar of community school programs and events.
• Coordinate program logistics, including scheduling, outreach, registration, supplies, and partner participation.
• Promote programs and resources through flyers, newsletters, electronic communication, school events, and direct outreach.
• Gather feedback from participants and use it to improve future programming.
• Identify, develop, and maintain partnerships with community-based organizations, government agencies, healthcare providers, businesses, faith-based organizations, and other community resources.
• Monitor the quality, accessibility, and effectiveness of partner-provided services.
• Establish clear roles, responsibilities, communication processes, and shared goals with partner organizations.
• Address gaps, duplication, or barriers in services and help partners coordinate their work effectively.
• Schedules talks at community functions i.e. PTA meetings, ANC meetings to make community aware of program;
• Meets with parents and family members to make them aware of available services;
• Engage key community partners in attending the meetings
• Solicit input to develop work plan and goals to achieve each year
• Manages meeting minutes and logistics
• Monitor and track the achievement of goals;
• Monitors referrals and progress towards achieving objectives;
• Implements corrective action methods when performance is below targets;
• Shares information with consortium leadership and incorporates in evaluation process;
• Ensures all documentation is up-to-date.
• Writes comprehensive mid-year and end-of-year reports for funder.
• Participates in program development and implementation of all phases of program.
• Possesses knowledge of the organization’s requirements/systems/reports/processes to guide practices and support compliance
• Possesses knowledge of internal programs, processes, and key contacts to facilitate appropriate referral and resolution of identified issues
• Ensures all confidentiality requirements are adhered to regarding participant services.
• Participates in development and monitoring of budget for project;
• Ensures ability to stay within budget for program;
• Supports the identification of additional funding to sustain the project;
• Helps to develop proposals and reports to secure funding.
